This little pachy may be trying to crest, one of her leaves is mutated and almost heart shaped and it's coming out of the center in a spiral growth pattern. Cresting occurs either genetically or when a succulent has had some sort of damage to the center of the rosette, if the damage isn't too bad it alters her tissues and this can result in a plant that starts to mutate and crest.
